Is transfusion of vcABO comparable blood more hazardous than ABO identical blood ?

No, it is not the truth. When you are transfusing  RBC , It should be saline wash RRCs than no harm to transfuse ABO compatible cells. and in the cases of plasma & Platelets AB is universal   donor. By this you will also manage excess or shortage of different ABO blood group. For this you should have well equipped Component separation setup including sterilized tube connecting device. We are issuing ABO compatible component to the patients safely.

We generally prefer transfusing group identical red cells however in dare emergencies where O donor unit will have to be issued, we commonly perform screening for the presence of alpha and beta haemolysins in the plasma of the O units, which could react with recepients' red cells that have the corresponding antigens.

No it is not the truth. When O cell saline wash RBCs is issued to other group patients it is quite safe . for that you should have well developed Component separation unit including sterilised tube connecting divice.. When AB group plasma issued to other group patients it is also quite safe. This will also help to fulfill the blood demand in emergency as well as help in maintaining the stock position of blood components Excess / shortage. It is the need of time , when identical group is not avialabl one should issue compatible RBC and plasma/ other blood components.

The answer as fellow colleagues pointed out, is NO, as long as proper compatibility testing and screening for high titre Anti-A, and B was performed before transfusion. Though in most clinical institutes, the transfusion guidelines advise using ABO identical blood, except in emergencies.
