Nonalcoholic steatohepatitis is a common clinic-pathological entity with a worldwide distribution. The disease is often indolent, but 15%-20% of patients may progress to fibrosis and 7%-17% may insidiously develop cirrhosis which is a strong warning finding in patients’ liver biopsies. Which factors can predict this event in NASH patients?

... at least in children and adolescents there seems to be an association of age/ pubertal stage with surrogate markers of steatosis and steatohepatitis. For more in-depth information, I'd like to refer to our paper "Gender-specific prevalences of fatty liver in obese children and adolescents: roles of body fat distribution, sex steroids, and insulin resistance" (The Journal of clinical endocrinology and metabolism 09/2009; 94(10):3872-81).

We had shown in our 2004 paper in Tropical Gastroenterology that males have a greater predilection for NAFLD. The prevalence was 13.8% in females in comparison to 26.9% in males [Singh SP, Nayak S, Swain M, Rout N, Mallik RN, Agrawal OP, Meher C, Rao MVK. It occurred to me that you might test for alpha-1 antitrypsin deficiency. Some alphas are at risk for liver disease and this susceptibility however minor could be a contributing factor towards other problems in your NASH patients!
In a publication by our group, "Does weight history affect fibrosis in the setting of chronic liver disease" JGLD 2009 We found age and length of time obese but not sex on univariate analysis to be of importance. On multivariate however age drops out and is replaced by the length of time one is obese. Severity in this case is defined by degree of fibrosis.
If you go to my page and look at publications it is availble as a full text download. In addition to above comments 15 years of obesity (BMI>30) or greater was a strong predictor of more advanced fibrosis.
