There is a book coming our soon edited by Steve Trenoweth that I have contributed to that covers this very subject. In the interim I would recommend reading chapter 14 on Assessment within the Norman and Ryrie book "The Art and Science of Mental Health Nursing; Principles and Practice. It is published by Open University Press.
