If I understand what you are asking - it would be the deconstruction of the key factors that are the ingredients of successful organizations and reducing that into a series of qualifier questions. Does this sound like what you had in mind - I would like to collaborate with you on that as I work with start-ups.

What is the make-up of your Board of Directors? How large is your Board and what do each member bring to the table? What is your strategy to promote from wihtin? How often and to what degree is training held? How effective is your R & D division? Over the past 10 years have you met your yearly objectives? How do you value your employees? How are your people people prepared for leadership responsibilities? Has you mission and vison stayed current Have your read 2010 Good to Great? You may want to revisit that book.
