While abstracting data on the extraction sheet. Should we have to copy and paste from the respective papers or we have to manipulate the data in our own words, particularly while documenting the result and discussion section?

There is not a particular data extraction sheet to use in a systematic review or MA. You may find examples if you search the web. Happy to provide you examples if you don't find anything that fit your needs.

Cochrane review book offer an examplary template for data collection;however, you can modify it based on your own protocol.
