If you use a new questionnaire with new variables, a pilot study is recommended before doing the large scale survey. The pilot study would allow you to identifies and fix inconsistencies and mistakes in the questionnaire. It's not "mandatory", but given the resources required for a large scale survey, you're better be sure the results you get are usable or don't have strong biases.
It will provide you with reliable data to test the quality of your questionnaire i.e validity and usefulness of the items contained in the questionnaire which is determined through principal components analysis / factor analysis.
The minimum sample may depend on your total population. However, I recommend 10% of the entire target population.
It is highly recommended. The pilot test is the understanding of your future analyses. It also helps to understand the instrumental reliably. In addition, it gives a proper understanding of your questionnaire.
For the pilot test, you can 30 or 50 or 100. It actually depends on your sample size.
