Because once you ask the name, the respondents becomes alert while disclosing income or many other personal data or opinion. If their names are not asked they feel free to give information or opinion, as per my experience. "The data will remain confidential" declaration remain always doubtful in the mind of the respondents.
To avoid bias and to maintain confidentiality it is better to keep questionnaires without asking respondents name

I have got the similar kind of recomendation from Prof. S.N. Dhar of North Bengal University, West Bengal, India.
Best practice is not to request names on questionnaires. This protects participants in case questionnaires are lost or misplaced so that answers to sensitive questions cannot be linked to an identifiable person. If funders or RECs or auditors want proof of enrollment, the separate consent forms can suffice. If for scientific or other reasons participants need to be linked to their data, the research can assign each participant to a unique code and a separate key file/document can be created that contains this link. This key shoud be kept in a secure place or password-protected file.
I am also doing a survey and when i put the name i felt people will feel bias in responding, so i removed the name.
It is not mandatory to ask the name. As you have identified, asking names normally leads to biased opinions and answers, if the survey questions are of critical in nature.
If you use name also in your data analysis then you can keep name in your questionnaire. I believe no need to have name of the respondent in the questionnaire... !

No, please. It is always good not to request respondents' names. This gives them the leverage to express themselves freely.
