Reading through reviews by Biffi and Greenberg (J Clin Neurol 2011) and Attems et al (Neuropathol Appl Neurobiol 2011), I could not find an answer. Is female sex overrepresented among patients with CAA (possible, probable or definite)? Is it a risk factor for CAA-associated intracerebral hemorrhage in humans? Thanks in advance for any references.

I see we are colleagues at the Karolinska, and I’m happy to see there are more small vessel disease enthusiasts at our hospital.
I'm looking for an answer to this question as well. We have shown an excess of cerebral microbleeds in males, both deep and lobar. Certainly small vessel disease, or this certain marker of small vessel disease is associated with male gender. However if this translates to a gender bias in CAA is unknown. We have an association with both deep (presumed hypertensive arteriopathy) and lobar (presumed CAA), and it may simply be so that since males have more hypertension they also have more hypertensive arteriopathy and that the additional burden leads to a higher degree of vessel fragility, causing more microbleeds (this is my own theory).
In our study on cortical superficial siderosis (submitted) supposed to be a sensitive marker of CAA we haven't seen any sex bias.
However it gets more complicated than that. ApoE4 has a female bias and so does Alzheimer's disease. Supposing there is an overlap between CAA and AD, CAA should be more common in females?
All these theories remain to be investigated and understood.
