If someone wants to do independent research (e.g. they design a questionnaire and ask people to take part, or they are a personal trainer and they collect data from their patients for a new exercise intervention etc) how do they publish the results if all journals require approvals from ethics committees and compliance with the Helsinki declaration?
I've heard to obtain ethics clearance from an independent ethics committee costs a few thousand dollars.
Is research publishing in a way "locked" for academic institutions only?

There is a UK/European focus - but also reference to an NZ arrangement.
Seeking 'approval' or an 'opinion' from a research ethics committee is the best way forward, since there are no 'sanctions' available for those not following Helsinki (or even Oviedo). Often researchers simply state that they 'follow' Helsinki - or RESPECT, ESOMAR or whatever is the most appropriate code/guidelines for their field of interest. No one should have to pay 'thousands' for a REC opinion - even then it cannot be considered an 'approval' unless it comes from the funding/commissioning organisation.
What matters here, ethically, is that you are seeking to do this in the best way!

The point of research is to publish/inform an audience. The way research is published affects how it is viewed by potential audiences. Academic publishing houses/institutions are careful about the research they publish. Academic readers do not have time to read research that is not carried out following disciplinary guidelines. Those guidelines ensure that the research is likely to pass critical scrutiny by reviewers and therefore may be 'worthy of publication'. Ethical approvals are obtained prior the conducting research and ensure that studies meet ethical requirements. Research without ethical approval is not likely to be reviewed and will not be publishable. Independent Research has extra overheads and ethical approval is one of them.
So, in answer to your question.. yes, in a sense it is locked because externalities (supervision, management and admin costs, research methodology, ethics approvals, meeting publication standards) are all internalised in the academic environment.
