for the first you'll need to have strong data and documented proposals. take a look at the "diabetes" journal and contact editors, ie martin Myers who is the editor in chief (http://diabetes.diabetesjournals.org/content/editorial-board). for the second, it's more difficult since it depends on your institution and your country, and it can take a long time and need some money, it's not free, far away.
