If I understand your question correctly, your query is about the points falling above the upper control limit.

If any point is falling above the UCL and below LCL it means the process is out of control.

First of all, I truly appreciate your answer and I understand it because back in the day I took some classes in TQM and Production Operations Management. Now, the question has a context in psychology (Lakens, et al., 2018). As far as I understand these authors, a range is established to evaluate if an effect falls between these lower and upper limit. If the effect falls within these range, it would be to small to have a practical meaning. If it does not fall, it would be large enough to have a meaning. I just want to see if these affirmations area correct.

Let's use a simple educational layman's language: when doing evaluation, test items are evaluated to check their capability to achieve a desired goal. First, the difficult index (p) is calculated by adding the correct answers in the high and low groups, then divided by the total number of papers analyzed. Secondly, the discriminatory index(d) is also calculated by subtracting the sum of correct answers in the lower group from the sum of correct answers in the upper group, divided by total number in either group. p is divided by d multiplied by 100 to get the %. The result should range between 35% to 65%but best around 50%. Beyond these, the test item is either too easy or too difficult and can not be used for evaluation.
