I read some suggested books regarding building qualitative interview questions but I am still lost and worried. I rewrite my interview question nearly 20 times but still do not know whether it is good or not. does anyone have any previous experience with this topic?

Have you looked at books that are specifically about doing semi-structured interviewing, such as Rubin & Rubin, Qualitative Interviewing: The Art of Hearing Data?
I have referred to this book and read the related chapters, it did help me a lot. but I am still worrying about my questions.
Hi. You need to ask some experts in the field to check your questions based on your topic and determine their content validity.
That depends on your research questions. Regarding the first 2 or 3 questions, they are normally about the participants' demographic information or your research context.
