I am pursuing a book project: The Sacrilegious side of the Saint. Apart from other issues, it will try to correlate single Christians' (people of faith) sexual behaviour with that of their spirituality to see the impact on their lives. Is sex a biologically necessity? What are the implications for the church's teaching on sexuality?
Its not about the design of the questionnaire, you should include a consent form that include something similar to the following:
"Your participation is voluntary. If you do not wish to participate, simply discard the questionnaire at any time. All your information including your name will be kept completely anonymous and will be used for the purpose of this research and destroyed after two years". If they read something and still have doubt then nothing else will or can make them change their mind !!

You might consider administering a measure of social desirability i.e. Crowne-Marlowe Scale:
Crowne, D. P. & Marlowe, D. (1960). A new scale of social desirability independent of psychopathology. Journal of Consulting Psychology, 24, 349-354.
The idea is that those who score higher are more likely to display social desirability thus either answering in accordance with (in your case) religious orientation; or their own perceptions of the results you desire. You may then eliminate thos participants with particularly high scores from analyses.
