i. Constructs reflect the research objective
ii. Developing items based on experiences, observation, knowledge, ideas
iii. Retrieve the items from literature review

The best way is to use standard instruments. If you want to create tools, follow the steps below:
1) Study the theoretical foundations of variables
2) Identify components
3) Identify items of each component
4) Calculate the validity items by content validity and construction validity (using exploratory factor analysis ) its better to use structural equation modeling
5) And then calculate reliability

Essentially, the questionnaire translated research objectives into a series of questions. in my PhD thesis, I have used the following Steps:
Step 1: Conceptualisation and operationalisation of the study variables.
Step 2: specifying the survey method.
Step 3: developing measurement scales.
Step 4: preparing draft instrument.
Step 5: assessing the reliability and face validity of the instrument.
Step 6: pre-testing the instrument.
Step 7: The instrument distribution, management and collection.
