In cases of severe zonular dialysis and marked lens decentration the Cionni MCTR is used to provide stability and IOL centration in the bag and also to allow phacoemulsification.
Insertion of CIONNI CTR is challenging as there are high chances of preplaced suture entanglement.
One trick you can use is that; once you preplace the suture from eyelet and start injecting CTR, try to keep suture somewhat tight (not too loose) by pulling it out from scleral flap simultaneously while injecting more and more part of CTR inside.
By this way you can make sure that there is not sufficient amount of suture to get entangled :-)
You can have an assistant holding the trailing suture and keeping it from entangling. Otherwise, keep it sufficiently long and then clamp it to the drape.
