I am looking forward to seek some help regarding adaptation of mature questionnaires. I am new to quantitative studies and wants to use some demographic in my questionnaire, is it necessary to add the source of demographic questions to justify them or we can use any questionnaire. I am planning to use following demographics. if anyone can help to improve please:

You may use Demographics that you feel have an impact on your study. For example, if you feel that you would like to measure "gender" against a specific question in your questionnaire, then you will have gender as part of the demographics. Hence, you dont have to over-saturate your questionnaire with too many demographics questions, but just those that you feel may have a bearing on your study.

Basically, a questionnaire can consist of two parts: (a) Personal/Demographic information, and (b) Research Information. The focus on the demographic information should be minimized as much as possible as required by your research objectives and compatible with research information. The design of a questionnaire can be eased with your study conceptual framework, if you can possibly make up such a framework. Also, it is somehow important to justify in your studies that the demographic variables have validity to appear in your questionnaire in accordance with your study variables. In this context, you can decide what demographic variables needs to placed in your questionnaire.

If a question is simple and has high face validity (such as requesting a person’s age or sex) I don’t think that it is necessary to cite a reference that also asked such questions. But if it is not obvious what response categories would best define the construct, then citing a reputable source would help justify the adoption of those particular response categories.
For example, it would be desirable to use the sample response categories that are used in published national census reports, for say, family income, or highest level of education. That would also allow an evaluation of how representative your sample is, by comparing the distribution of demographic variables in your sample with that for census data.
Just a couple of comments on your proposed demographics items: For age, it would be simpler to ask for their age in years. But if for some reason, you think that people would not wish to reveal the exact number of years, at least make your categories the same width, so that the scale will be equal interval. Then you will be able to examine non-linear, or interaction effects.
For education, I assume that you would want to know what their highest level of education is, and I would introduce the question with wording to make that clear. Otherwise some individuals might select, say, both a Bachelor’s and Doctorate degrees.
For questions 9 and 10, those response options would not be able to distinguish between numbers of children or family members, between 2 and 4, or between, say, 5 and 8. Those differences could be important. So unless there is good reason not to, I would simply ask the participant to enter the actual numbers.
