Questionnaire development is a rare skill that dread even seasoned researchers. If you cannot find a developed one that suits your research framework with minimal customization, you will have to develop one. The starting point is your research conceptual framework (constructs) and then develop the items/questions which describe the constructs based on literature from authoritative scholars within your research topic. The deigned questionnaire can be validated through expert interviews with researchers and practitioners knowledgeable of your topic including fellow doctorate students. You can pilot test the questionnaire to improve its construction. These processes can be iterated to perfect the instrument.
Thank you all for your suggestions. In many circumstances the questionnaire that we adopt from other researchers from a country do not exactly fit in to all of the places. I find it tedious yet fascinating that questionnaire development become a different research inside a research.
