From my observation, the ECV values usually covers >90-99% of the isolates, and the literatures also state that strains that have MICs more than ECVs are considered non-WT because they have resistant genes. But I never read any articles mentioning that they confirmed those non-WT molecularly to have these resistant genes, or the ones considered WT definitely do not have these genes.
