Extensive review of related literature is very important. While thoroughly studying the related literature make a note of the areas covered by the earlier studies and the points that you want to cover in your questionnaire. Work on developing different section for the questionnaire then in each section you can frame the related question. Try to add different types of questions such as rating scale, multiple choices, and likert scale etc. i.e. the ones that will aptly serve the purpose of your study. All the best.
Start with hypothesizing how the final draft of the study might read.
Questionnaire research usually proceeds along fairly cook-book standard lines; item construction, testing, factor analysis, deletion of items etc.
What is often spectactularly missing from questionnaire development and research is validation against other-source data and tests of common method variance. If you really want to do new research, try that.
Actually, you could start by asking whether there are other methods which could be used to investigate your problem. If so, will they yield the same kind of results as a questionnaire? If not, why so?
My general point is that questionnaires are often very unreliable, and prone to systematic bias, which results in common method variance.
