How do I gain access to the Screening Assessment Tools, the Desire for Drugs Questionnaire (DDQ) & the Obsessive Compulsive Drug Use Scale (OCDUS)?

I haven't been able to find the actual forms for either of these tools that contain both the questions and scales. I've found the questions, but not the values of the Likert scales used.
