Primary health care approach is widely adopted in almost all countries. It is essential and growing. Public health activities need some sort of augmentation. One proposal to make use of primary health care settings to help better public health implementation.
In the United States, there is a big push to integrate healthcare and population/public health. The two sides have much to offer.

Omran S Habib , in the United States, the two traditionally strange bed fellows are beginning to fall in love through national initiatives. The non-profit hospitals are, by regulation, required to work with public health agencies to conduct community health needs assessments and incorporate a component of population health. So the answer is that it is difficult but possible through proper policy and regulations.
The community level or local access is paramount for general medicine (bottom-up); it is also a fact that specialized clinics are most efficient. So, sometimes further travel will be required for better treatment.
I think addressing the Social Determinants of Health with each patient treated and the environment of the patient does a lot toward individual health and public health. We need to be involved in direct patient care and in the development of policy that address the Social Determinants of Health.
Home Care, Community Health Services, Diseases Monitoring and Control Programs, Epidemiology and Statistics in PHC settings, Communicable Diseases Monitoring, Disease Prevention in PHCs, Immunization in Primary Health Centres, All are part of Public Health Activities in PHCs. It should be Promoted more in a Midern Advanced Methods so its more Productive and Successful. Regards
