medical negligence is due to the number of factors, employee factors, for example, long working hours, lack of motivation, stress, incompetence, family problems, and some times just poor attitude, it could because of poor communication between doctor and nurse, some nurses feel so inferior, to consult doctors for medical reports they do not understand, why work like a robot , claiming that " the doctor said so", even when she very well knows that the dose or medicine she is going to administer is nt right,.......for example, a nurse may end up injecting a particular drug to a neonate, yet it is meant to be given to the mother...
I think the comment by Sandra Agondeze is on track. Medical errors are a subset of adverse events to patients; and adverse events, including medical errors, are usually multifactorial. It requires careful systems approaches by hospitals to reduce the occurrence of adverse events and medical errors. See the attached file.
I agree, but there is more to focus on when speaking of med- mal. These are issues that cause the majority of malpractice laws suits; they are treatment procedures. Treatment procedures should have specific protocols and best of all; checklists. Protocols should be reviewed prior to the treatment. If equipment is involved, especially equipment the patient is connected to, check it with a partner-once again a checklist is a safety tool. Slow the pace down to make sure all systems are a go. Have one person-not the physician- give the go ahead to proceed. This helps prevent the staff moving ahead when they are not sure all is well. Use the timeout. Administration has to promote and uphold the timeout with and for the staff. Remember the physician is not the only one who can be suited. It takes a team to support and promote patient safety.
