As you know, different distribution of HCV genotypes is observed worldwide. On the other hand discovery of IL28B SNPs and its distinct distribution by ethnicity has changed our insight to hepatitis C natural history and treatment. Also, many studies showed the association of IL28B SNPs and HCV genotype in chronically infected patients. Altogether, can we search for the impact of host genetics on distribution of HCV genotypes?

It is very critical point to attend, is any relationship between the different genotypes of HCV and spontaneously clearance of acute acquiring HCV infection of not? if the response is positive, can we conclude that the host factors can change the epidemiology of HCV infection during long period in the community or not?
So far,IL-28B genotype is mainly related to favourable treatment response with interferon rather than spontaneous viral clearance or susceptibility to HCV infection.Although the role of the same genotype appeared not that crucial in HBV or HIV infections,there is definitely a need to understand the role of both host and viral genotypes for epidemiological consequences of HCV infection with time for particular ethnic community.
You have raised two questions here, both highly important. The possible impact of host genetics on epidemiology of HCV genotype is a hot topic these days. For example, there is evidence that genotype 6 is highly prevalent in East Asia and Sout-East Asia. We also know that IL28B CC genotype is highest there. One explanation might be genotype 6 being spreading more efficiently in people with IL28B CC genotype.
Your other question about the relationship between the HCV genotypes and spontaneously clearance is another hot topic now. Initial studies have were conflicting in this regard. However, we have recently published a study on a large population of people with acute HCV and demonstrated higher rate of spontaneous clearance in genotype 1 compated with genotype non-1. Interestingly it is in contrast with repose to treatment where genotype 1 is associated with poorer response. Look at the paper through the following link:

I am a bit hesitant about that part of the comment by Rathindra Mukherjee about "IL-28B genotype being mainly related to favourable treatment response with interferon rather than spontaneous viral clearance". There are strong evidence indicating IL28B genotype as a major host factor associated with both spontaneous clearance as well as interferon induced clearance. Thank you for your contribution. What is the role of INFL gene variants in hepatitis C spontaneous clearance by the HCV genotype? As you know these genetic variants have significant impact on treatment response of HCV genotype 1 and 4 while their impact on the treatment response of HCV genotype 2 and 3 is not proved yet. Can we state that they act the same in spontaneous clearance of different HCV genotypes? is it going to change the distribution of HCV genotypes around the world?

In our data, the interaction between HCV genotype and IL28B genotype did not reach statistical significance on the multiplicative scale but it is an important topic which requires further investigation. In our analysis we had to pool all genotypes non-1 due to small numbers. It means that we were not able to assess any difference between various genotyeps non-1 in terms of prediction of spontaneous clearance and also interaction with IL28B.
