I would not agree, however, that herbal remedies are inherently safe. All drugs, no matter where they come from, have a therapeutic index, the ratio of the dose that causes adverse effects to the dose that has the therapeutic effect.

The art of the Chinese medicine is not to kill the intruders. Instead, we boost up the patient's immunity, change the patient's internal environment to not fit intruders to survival. In Western pharmaceutical research of the type in which I am employed, the goal is to identify a single chemical substance (or sometimes a well-defined mixture of two substances) that has the desired benefit to the patient, and for which the toxicity is acceptably low. It is not expected that the drug is absolutely harmless at any dose, but it is necessary that the potential for harm at the therapeutic dose is outweighed by the potential benefit. Therefore, controlling the dose is a crucial aspect of the therapy. That is why pure active pharmaceutical ingredients are required, and why there are strict controls on the manufacturing process.

With herbal remedies, it is very challenging to assure consistency of dose, since the amount of the active substance may vary from plant to plant, and depend on how the plant was processed and stored. This problem has hampered attempts to conduct controlled clinical trials on herbal remedies. No doubt experienced practitioners of Chinese herbal medicine are able to deal with this problem through their training and experience, but the same could not be expected of the thousands of doctors, nurses, and pharmacists without such training and experience.

Hypothetically, if you were to tell me of the existence of a plant that, according to Chinese medicine, was effective in the treatment of bacterial infection, my goal would be to isolate the active chemical substance responsible for the effect. This substance would be called a "natural product." (Many of the antibiotic drugs used today are natural products, or derivatives of them, that were originally discovered in soil samples containing bacteria that produce the compounds, or a certain mold.) The chemical would be prepared in sufficient quantity for testing, either from the plant or, if possible, by chemical synthesis (The ability to chemically synthesize the compound would allow variants to be made and tested, as well as the original substance, and avoid over-harvesting of the plant). There would be a series of increasingly complex and expensive tests in the laboratory designed to establish the suitability of the chemical for the therapeutic purpose and its probable safety. If the chemical passed all of these tests, it might eventually be tested for safety in healthy human volunteers (Phase 1). Only if it passed that test would it be possible for it finally to be tested for efficacy in patients with the disease (Phases 2 and 3). The whole process would take several years, involve hundreds or thousands of people, and cost many millions of dollars.

Let us suppose that the pure chemical was deemed to be too toxic for use in humans at the needed dosage. Would that result in the original plant being banned from use in Chinese herbal medicine? This seems unlikely to me.

As the first case that I pointed out is the other supplemental industry caused the Ephedra to be banned. The second and the third one, I did not trace when and why.
