You must have the end game in mind - what you want to attain in 3-4 years' time of the PhD journey. The concept paper must be clear from the beginning and you can use the Delphi technique (panel of experts) to generate ideas to a problem. You then develop the first chapter based on the problem statement and literature review and thorough understanding of what the theory says - this assumes you are opting for a deductive approach.
The research proposal is now set which includes Chaptets 1-3 (Introduction, Literature Review and Methodology). The methodology is yours but you may adapt what others have used in prior studies.
The ball is then in your court - Chapter 4 (Data Analysis and Findings) and Chapter 5 (Conclusions and Recommendations). Then defend your Thesis.
