I'm looking for a very short feedback instrument/score for a faculty development course on feedback (in the context of workplace based assessments) (usable for pre-post, a rather small n, max 5 items).
Does anyone know of such an instrument?

the most simple feedback instrument in pedagogical task (maby in coaching too) is the video.
Feedback Orientation Scale (FOS): The FOS is a 6-item tool that assesses an individual's orientation towards feedback. It covers areas such as receptivity to feedback, feedback seeking behavior, and feedback giving behavior. This tool can be easily adapted by selecting a few items that are most relevant to your course.
