Hi everyone,

I had a patient 3 days ago complaining of left ear fluctuating sensorineural hearing loss, otofonia before attacks, and vertigo(lasts one hour).

his test results :

unterberger has no deviation romberg normal oculomotor tests are normal

there is no spontaneous nystagmus.

But in post head shake test there was left beating (5 spv), and downbeat nystagmus(9spv).

And in the caloric test, there was no asymmetry (%15 right canal paresis), surprisingly left irritate more.

Why there is a downbeat nystagmus and why ı see left beating nystagmus?

I was waiting for right beating Nystagmus..

I guess the patient will have an attack soon, Is this guess correct?
