Does anyone know of Questionnaires/measures about adult knowledge and attitudes of/towards mental health in children and adolescents?

I am looking for measures used to assess the following domains:

1. Attitudes/beliefs held about mental health in children/adolescents by adults

2. Knowledge in adults about mental health issues in children/adolescents

More specifically, I would like to explore these domains from the perspective of school teachers as part of an exploration of mental health literacy in teachers (in terms of child/adolescent mental health). Any assistance would be greatly appreciated!
