I am developing a pilot research project where I need a mobile application that can be used to collect health-related quality of life data through a questionnaire and having the data saved in a cloud-based database. Therefore, I need to collaborate with such a company or a developer who is interested in the field and would like to be involved in the paper and further funding proposals.

We are using Filemaker Pro for developing and managing our patients' data. You can deploy your database apps on desktop and tablet computers. It's very easy to use and doesn't require coding skills.
