One of my collegues (dr Andrea Tramarin) worked in this field: I've found his paper published
Qual Life Res. 2006 Apr;15(3):377-90.
ISSQoL: a new questionnaire for evaluating the quality of life of people living with HIV in the HAART era.

You could use the Medical Outcomes Study for HIV (MOS-HIV). It was developed and validated in the pre ART era, so there are problems, but it gives a mental health summary score and physical health summary score, which are useful.
The SF-36 Scale is another one you can use. The SF-36 is a multi-purpose, short-form health survey with only 36 questions. It yields an 8-scale profile of functional health and well-being scores as well as psychometrically-based physical and mental health summary measures and a preference-based health utility index.
MOS-HIV Health survey is a good one. This is a disease specific version of SF 36. We get total scores, ranging from 0 - 100, for 10 different dimensions of HRQoL, and overall Physical and mental health summary scores. For four dimensions, the items are same as SF 36. Hence, we can compare the HRQoL of HIV population with general population if normative values are available. However, it doesn't have items related to medication concerns, sexual life issues, and stigma and disclosure issues
PROQOL by the Paris group is another new one in the field. We have developed one more psychometric scale in our institute which is already validated among the HIV positives.
The WHO QOL-HIV BREF questionnaire on quality of life has been developed by WHO for Asian region . It has been validated, reported and published. It has 31 item BREF-f version and 100 item long version for PLHIV. This measures quality of life in six domains.
A escala HAT-QoL é um instrumento que avalia a qualidade de vida de pessoas com HIV/aids. Ele foi construído especificamente para essa população. Possui 42 itens.

The PROQOL-HIV is probably the most recent HIV-specific health-related quality of life scale (I am a co-author). It was validated with patients in 9 countries across 11 languages, including Brazil in Portuguese (co-author Dr Mauro Schechter, Federal University of Rio de Janeiro, Brazil). It has been validated in both paper and electronic formats. There are separate publications available on the scale's development, psychometric validation, and paper vs electronic validation.

We have used the WHOQOL-HIV BREF for our study on PLHIVs . This has been used for across sectional study in India and published. The six domain analysis is useful.
We published a small review on instruments used to measure quality of life in people with HIV/AIDS. The SF-36 and the MOS-HIV turned out to be most widely used:
