I have gained a lot of experience with TOPO recently. I think the main error, which i had, is making sure your polymerase is compatible with your type of TOPO kit. For example, your insert must have at overhangs if your using the TOPO TA kit. I think the issue will be due to incorrect kit/polymerase. Hope this helps.
