I collected my data from three medical settings in one phase time where I conducted non-participant observation (i.e. medical consultation was recorded), then a questionnaire that patients had to fill.  In the questionnaire, there are qualitative or embedded open-ended questions.  I collected documents by the end of my fieldwork. My question is that:

Can I say that I used two mixed methods, namely convergent parallel and embedded design?  I want to make sure that I am using the correct designs?

I would be so grateful if any expert mixed method research can help in this regard? Am I using the correct design? or other designs are better than the ones I have used? From my readings, I found these might be the best designs to be used? What do you think?
