Dear Adebisi, an Interesting project you planned and as a comment, you can deliver appointment based clinical service for other chronic cases like hypertension with its comorbidity, heart failure, chronic kidney diseases, diabetic malitus, chronic coronary diseases etc.

actually appointment based service delivery is termed as medication therapy management which is supposed to be part of clinical pharmacy service and this appointment based service for a patient could effectively be given if you are able to develop software system which comprises patient name, id, type of cases and comorbidity, type of drug that the patient is taking, type of drug therapy you solved, laboratory parameters that should be done during patient visiting together with vital signs (BP measurement, heart rate, respiratory rate, temperature) blood sugar label, glucometer. once you develop this system in a software, you will easily capture during your patient appointment day and you can intervein according to specific problems ,gaps you will find up on your patients at their respective visiting day.
