Diabetic dermopathy is one of these many health risks for those with diabetes, and may effect patients of all genders, ages, and races if diabetes is not well monitored and cared for. There is, however, a correlation between the presence of skin lesions and the number of microangiopathic complications (retinal, neuropathic, and/or nephrogenic) present.
One cause of microangiopathy is long-term diabetes mellitus. In this case, high blood glucose levels cause the endothelial cells lining the blood vessels to take in more glucose than normal (these cells do not depend on insulin). They then form more glycoproteins on their surface than normal, and also cause the basement membrane to grow thicker and weaker. The walls of the vessels become abnormally thick but weak, and therefore they bleed, leak protein, and slow the flow of blood through the body. Then some cells, for example in the retina (diabetic retinopathy) or kidney (diabetic nephropathy), may not get enough blood and may be damaged. Nerves, if not sufficiently supplied with blood, are also damaged which may lead to loss of function (diabetic neuropathy).

Diabetic dermopathy is a frequent manifestation of microvascular complication due to diabetes. The increasing of collagen crosslinks and of the advanced glycosylation end products are probably the major cause. Therefore microvascular pathology might generate skin dystrophy/atrophy and neuropathy with loss of sensibility and impairment of skin protection against infection and difficulty of re-generation, features linked in the phenomenon so-called diabetic foot (a major complication of diabetic dermopathy - marker of microangiopathy).
