Recent Decades most of long term Patient care is being carried out at patient home. This gives rise to many challenges to provide quality care as per patients condition. we are caring in patient home not in hospital setting. That make lot of difference. In conditioned settings like hospital we have easy access to communication , policies and procedures but at patient's home the environment may not always conductive for care. that leads to exposure of patient to many risk factors . Increase risk of falls, injuries, infection, allergies and many more. Nurse's first priority is to ensure safety in any setting patient is been taken care of. thus I need help from expert that will guide me to formulate tool for Nurses to assess risk factors in patients home.
