Most children infected with the new coronavirus show few signs of illness, if any. But a few children are struck by a severe form of COVID-19 that can cause multiple organ failure and even death. Now, scientists have begun to tease out the biology of this rare and devastating condition, called multisystem inflammatory syndrome in children, or MIS-C.

It may be that the dominant T cell type in early life is more likely to reject the virus. Studies have reported that children produce a type of T cell called Th2, which can prevent the uncontrolled inflammatory response to sars-cov-2. Perlman doesn't support the virus's effect of biased Th2 cells on infection, but he does agree that there is a problem with immune overreaction.

Multisystem inflammatory syndrome in children (MIS-C) associated with COVID-19 is a severe presentation of SARS-CoV-2 infection in pediatric patients that has many overlapping features with Kawasaki Disease (KD). But in contrast to KD, in which elevated IL-1 plays a major role, in MIS-C levels of IL-6 are elevated in the absence of increased IL-1. MIS-C is a newly recognized cytokine-mediated presentation of SARS-CoV-2 infection.
