i am a paediatric nurse by background currently in school doing my MN-NP and involved in a community initiative to prevent childhood type 2 diabetes through MD outreach as well as afterschool programming
the MD outreach involves getting providers to screen women of childbearing age for type 2 diabetes and referring them to diabetes-prevention program and also screening children for obesity
After school programming involves holding healthy lifestyle sessions for elementary/ preschool children and families
the community is predominantly newcomers/immigrants who are highly education but without a job -- I would really appreciate any ideas re making this initiative successful

you can find some very exciting initiatives being implemented in several European countries searching for BEST practices in Chronic Care, in the CHRODIS Platform.
The CHRODIS Platform is one of the main deliverables of the JA-CHRODIS (see link below to the best practices in Diabetes management).
JA-CHRODIS is a European collaboration that brings together over 70 associated and collaborating partners from e.g. national and regional departments of health and research institutions, from 25 European countries. These partners work together to identify, validate, exchange and disseminate good practice on chronic diseases across the EU Member States and to facilitate its uptake across local, regional and national borders. The focus is health promotion and primary prevention as well as the management of diabetes and multi-morbid chronic conditions.
The ‘Platform for Knowledge Exchange’, includes both an online help-desk for policy makers and a clearinghouse providing an up to date repository of best practices and the best knowledge on chronic care.

Dear Meena,apart from physical activity it could be worth to focus on Vitamin D-levels and supplements if necessary. It is a combination of many things to consider. I have worked with pregnant women on this topic for the past three years and found that gestational diabetes (and preeclampsia for that matter) is found more often in women with low vitamin d levels 25 OH. Maybe Bruce Hollis and Michael Hollinck have covered your specific topic in their numerous and valuable studies as well. Good luck and please return to me if you find a hint.
