We are planning to implement a Triage system in our local E.D/A&E to improve the quality of care and to organize the work in this high-flow department.
The triage system that I have felt has the highest correlation to the disposition of the patient while maintaining the high interrater reliability is the ESI (Emergency Severity Index). Manchester triage has been blamed for problems which are actually organisational capacity problems but it actually works very well when you follow it properly. It was replaced by streaming in many places which have had little or no effect and cost more to use.
