I am doing a reserach on kidney stones where I am trying to correlate a participants dietary intake and kidney stone occurrences. I am trying to find a universal sample on how to develop a questionnaire for participants to fill out based on their dietary intake. Can anyone provide me with a sample of a questionnaire they have used before to collect participants dietary intake data?

There are these food frequency questionnaires - there is also some syntax (for SPSS) so you can easily calculate fat, fibre etc.
