Determine the rights you wish to request: Be specific about what you will be doing with the copyright holder's work, such as using it in your dissertation research

Contact the copyright holder or license agreement holder: Reach out (email) to the person or entity holding the rights to the questionnaires. You can use sample permission letters as a guide, but it's essential to create your own email to request the permissions you need.
