Are there some circumstances where is possibile to perform a test U with an approximation in Chi quadro?

i know that while conducting the Kruskal-Wallis test, the researcher should keep in mind that if the number of groups exceeds the value of three and if the number of the observations in each group exceeds the number five, then, in such cases, the sampling distribution is well approximated by the chi square distribution. However I don't understand the sense to use a Chi quadro approximation after a Mann–Whitney U test.

Specifically, in this research there are five conditions analyzed individually, considering the effect of the two independent variables, “sex” (M AND F) and “diagnosis.” (HEALTY AND PATHOLOGICAL). Which kind of non parametric test we will use? Paired or Unpaired?
