We would like to use this as an outcome measure in an early intervention for people with a range of musculoskeletal conditions. Ideally it will be brief to reduce participant burden.
There are many questionnaires/measures of topics of your interest. Oswestry works fine with disability and Tampa questionnaire about kinosiophobia and fear-avoidance. CPAQ is a proper instrument for acceptance of pain.

At the moment we are using Orebro screening questionnaire, but we understand that it may not be sensitive enough to detect change in function after an intervention. Has anyone any experience with this?
