A 14 years old adolescent with cirrhotic liver was presented with a severe attack of hematemesis. After resuscitation for 12 hours he was hypotensive, severely acidotic and with uremia. Can we proceed for endoscopy at this point or continue resuscitation firstly?

Are you sure of the diagnosis of cirrhotic liver? Seems young for that as opposed to biliary sclerosis or some other dysfunction. And, given the brief history ... why need an endoscopy if you're confident of the diagnosis. It won't speak to the liver necessarily but esophageal varises, perhaps. Whatever, it seems resuscitation is going to be the first order of business, and then exploratory exams once stable.
