I know of an adult patient who started to experience mild, reappearing pain in that area (only around the right epididymis/spermatic cord) soon after finishing a course of ciprofloxacin tablets. The pain usually appeared only when the area was subjected to light mechanical stress (e.g. pressing on the area, scrotal contraction during ejaculation). Physical examination didn't show any anatomical abnormalities or signs of inflammation, and urinalysis was completely normal. Since the symptoms didn't bother the patient much, no further investigation was undertaken. So can ciprofloxacin be responsible for this?
It depends on what the Cipro was given for. Such a patient should probably be taught to take his time when urinating and not rush or push. If Cipro was given for sth that also caused stress, the patient's frequent urination and effort to urinate by pressing could cause these problems - theoretically
Quinolone drugs actually can often have an analgesic effect. Perhaps the quinolones masked neuritic pains that became more evident upon cessation of antibiotic therapy.The pain that occurs after mild pressure is allodynia - evidence of neuropathy. Orchalgia, epididymalgia and painful ejaculation are common occurrences in men with pudendal neuropathy.
Did the man have pyuria or inflammatory cells in the prostate secretions? Was ciprofloacin given for symptoms or actual objective infection.
