My question is: I have designed all questions with the support of existing literature. One of my seniors has commented that I should refer to some theory or model to develop this questionnaire. I should have connected the questions with variables of that model.
Now I am confused about whether I am doing it right or wrong? and if she's right, then which theory I should adopt in my case?

I think your senior suggested you to choose a main theoretical models elaborated for e-learning and blended learning and to design the questionnaire according to the key concepts of the model.
I cannot understand from your questionnaire which is the theoretical model you have adopted so far. If you explain the questionnaire background, it would be easier for the RG community helps you :)

Thank you Francesca Amenduni for your support. actually I have designed this questionnaire with the intention to analyze the "design of courses" before they can actually be offered to students. So that we can recommend them to some particular type of students.
Now I haven't found any questionnaire from academic journals till now, which I can use to collect information from teachers. That's why I had to design this questionnaire.
I am studying models to relate to the concepts of the questionnaire. I have found the "Anderson online pedagogical model" as a relevant model to this survey form. This model describes the interaction of actors (e.g students, teachers), different activities which can be performed (as in my case reading, writing, etc) using internet-based options (as in my case video conference, audio, digital books, etc).
But as a beginner, I need some expert advice on choosing Anderson's model for my study.

Sure e-survey developed and is used widely nowadays because it is economic in saving resesrchers and participants time and efforts? It is unlike paper_ baded survey it consumes efforts and time..
E-survey can create charts and diagrams by it self, the thing that eases the analysis of the results.

In my opinion, you can adopt or adapt other developed instruments regarding your field. Try to search the instrument, u may get the permission from the develope. Designing instrument is a long process to make sure the data collected is valid. in developing instrument, we need to follow model or theory.
You can develop a very good questionnaire, which can include preferable site for the teachers learning, total time spent for learning, preferable site for learning (free or Paid), etc etc
I looked at the survey. Have the respondents take it only once. People do not have the time or patience to take the survey several times for each subject they are teaching? As to a theory, look at VAK theories of learning. That would seem to go with your questions.

I was asking about the theory for questions, which I have asked. should I worry about it, that why I have asked these questions? because I haven't found such a questionnaire designed for my research purpose.
