To keep it very simple I would always Recommend you to do a single sitting endodontics on a tooth which is vital.

The clinical presentation which u have described is mostly of a tooth which is non vital so doing a single visit endodontics would rather NOT BE ADVISABLE as it can lead to the failure of the treatment 

The question is how fast do you want the sinus to heal, how early do you want to stop medication and how early you want symptomatic relief for the patient? If the answer is early, I feel you should go in for a multiple visit root canal therapy, irrigating the canals well each time. In single sitting how would you, probably, achieve adequate drainage of infection? Further, would you be able to achieve sterile root canals? 

Single visit RCT may be good for cases with pulpitis and best with cases where you are doing a purposive RCT or there is an accidental exposure. There is some evedance, that single visit RCT is successful in select cases of periapical cysts. However, i won't advice you to go in for single visit endodontics in present scenario for teeth with periapical infection.

Single visit RCT for non vital tooth with periapical infection is not successful  and it tales some time to subside the infection and also many factors are involved in it.

Ankur Sharma has said it all. I believe that  attempting a single visit procedure in the clinical scenario you painted would amount to heroism. However, I would love an endodontist to comment on this.

Although in the presence of a sinus tract it is often advisable to place intracanal medication and wait for the signs of sinus healing, if the canal can be disinfected and dried I see no reason why a single visit RCT would not be as effective. Many endodontists, myself included, do a single visit RCT in non-vital cases, including sinus cases. There is not much in the literature that supports multi-visit RCT, and I am excluding expert opinion.  Multiple sinus tracts could be a sign of root fracture, at this case waiting for the sinus to heal might spare the patient a cost of a RCT.

There is literature to support single visit RCT in the clinical scenario.. As there is an alternative path for drainage there will not be any flare up if we complete in single visit. But I personally feel it is better to do such cases in 2 visits.. Since, the tooth is non vital with active infection present, I usually give a Caoh dressing for 1-2 weeks and obturate it in next visit.. I have noticed that the healing of sinus tract / pa lesion is better when following this protocol...

There is much in the litaratere that supportsmulti or single visit RCT. But these two microorganizm (E.Faecalis,and C.albicans ) are reported to be responsible for endodontic failures. Olso İt is often advisable to place intracanal medication (Calcium hydroxide ) and vait two week.

Single visit RCT is not adviseable in this case as infection would take time to heal. And yes above mentioned statement by Beyser Piskin is very true that you may require intra canal medication for couple of weeks and you can only obturate until u get ur paper points dried and most importantly without any foul smell.

i can say if you can achieve a dry canals after excellent root canal preparation & irrigation, go for obturation. the sinus tract will resolve .....

Conventionally, there were 3 indications for obturating a root canal i.e.

1. easily dryable canals

2. painless tooth

3. healing of any sinus if present at the start of treatemnt

But now when single visit root treament is considered having more adavantages over multiple vist, single visit obturation in the cases as with chronic abscess may be safely performed if the dryness of canal is achieved. And when I say ' Eeasily dryable canal" it means a canal which becomes absolutely dry using 1 or 2 absorbent paper points.

If the canal is dry at the time of obturation and you have followed protocol with proper chemomechanical cleaning then I see no point in doing multple visits......other than having the chance to re-irrigate, but in a root canal environment that now might have MORE bacteria than at the end of the first visit, and now it is those hard-to-kill-bacteria that rule. Reason being is that there is variable efficacy of Ca(OH)2 to reduce bacterial counts, like: the physical barrier of the biofilm, the extracellular matrix of biofilm-embedded bacteria, anatomically complex areas such as anastomosis, lateral canals, isthmuses, irregularities, and dentinal tubules, inactivation and buffering effect of dentinal matrix, seepage of exudates including the proteins such as albumin through the apical foramen or dentinal tubules, different susceptibilities of bacterial taxa to the antibacterial effect of this medicament , and different types of bacterial strains in the tubules of root dentin, which may escape the direct action of high pH of Ca(OH)2.

If I have understood the litterature correctly then it all depends on wheter the root canal has (cultivable) microbes after the chemomechanical cleaning during the first visit. If the canal is indeed culture positive, then you will actually have increase of bacteriapositive root canals (from 24% to 71% - Peters LB, van Winkelhoff AJ, Buijs JF, et al. Effects of instrumentation, irrigation and dressing with calcium hydroxide on infection in pulpless teeth with periapical bone lesions. Int Endod J 2002;35:13-21) between the two treatment sessions, and an increase in the bacterial load - (from 1.8 x 103 to 9.3 x 103) mean cfu mL-1 counts between the two treatment sessions. If the canal is culture negative, then why not fill in one visit? And if it is positive, then the cell-count might increase.....so I think the whole thing is the other way around.

Erik you are very right multiple visits pose more problems and a canal that is dry at the time of obturation, means it is free from microbes.The periapical infection will resolve in due course of time as the cause has been removed.
