Proper specification for materials and verify them at site.If it is concrete related construction QA/QC manual and staff must. Any payment need to support by quality certification. Any way this is big scope , most of the plans do not work at site.

Cost , Time , Scope and quality are key variable , if one change others may affected positively as well as negatively. Better fix base line before start with price. Contractors very smart to play with quality. If you need quality job get reputed contractor who can assure quality no need much with quality control by you. As a example if you buy BMW,Toyota car are you worrying about , durability safety so on, it is not insult but think about Indian , Chinese Car , may be good , may be not who knows

Maintaining quality implies there is a quality plan written for the project, in this light you have to read through the document to understand what is required at each stage of execution of the project with respect to materials quality and workmanship as stated or expected for the project. In the absence of such quality plan, your expertise and that of your team will come to play; you will have to define what quality you expect based on standard requirements for materials and workmanship. Also, you will have to look at your construction methodology to see how this will influence the quality output and how this quality can be measured and maintained.

You have to prepare proper inspection and test plan and prepare inspection checklist for each activity of the construction according to your contract specifications, drawings and site situations.

Inspection checklist for the construction activities will help you more for maintaining quality at your project.

Before failurity happen we need to undertake and check the quality of material as per specification. In addition to that the design must fulfill the minimum code requirement. Based on failurity type different remedial action will be under consideration.

In Portugal, we rely heavily on inspection residing on site and control all activities from foundations to finishes.

The person who is in the work should have a deep knowledge of the materials but essentially in the project

The key is understanding the specifications but more importantly the background behind the specs. I've seen the wrong things specified before due to a lack of understanding. An example would be stainless steel bolts and nuts where vibration is prevalent. Without double nuts or lock nuts, which weren't specified, the nuts just vibrated loose.

Making sure all involved understand why the spec is like it is, so corners are not cut, just to save a little money. Just one short cut can be a disaster in the wrong place.

Finally, keep an eye on every step of the process, to make sure that the spec, the right one, actually gets installed. It is easy for the installers to have 'a similar ' product supplied to them, but this might not meet the same criteria.

To sum up. Planning, understanding, explaining and diligence. Patience and not succumbing to being rushed also help.

Quality construction is possible by adopting proper record keeping, inspection, testing and implementation of codal provisions at site. Many countries have their own codes for construction practices and one has to ensure these practices at site. The role of site engineer is very important in order to ensure that construction is done as per drawings and specifications.

I agree with Om Prakash. However, building on this, the points Om mentioned should be captured within a inspection and test plan (ITP). This is a document frequently used by UK construction companies, prepared at the start of the project specific to each construction activity i.e. brickwork, drainage, earthworks, etc. To prepare the document the project specification and relevant standards are reviewed to determine which points are applicable and if hold points are necessary to allow inspection/sign-off. From the ITP, task specific checksheets should be produced to capture in detail the quality requirements which can be signed off as an activity progresses i.e. for earthworks: number of roller passes, fill layer thickness, finished surface tolerance, etc. Checksheets are a form of record keeping and a means of signing of work. The ITP is a high level document.

AUTODESK produce a cloud based application entitled 'BIM 360 FIELD' which allows good record keeping in the form of daily diaries and production/sign-off of checksheets via mobile devices such as Ipads. This would allow paperless quality control. ITPs should still be paper based documents kept on site.
