Why would you assume necessarily equal proportions? Take a look at WG Cochran SAMPLING techniques, 3RD Ed remember that all samples aren't simple random samples. Best wishes David Booth

Use the proportion of treatment outcomes and the Wang and Chow (2007) formula for the calculation of the sample size.
