Regarding unilateral tinnitus in a patient complaining of bilateral mild symmetrical high frequencies SNHL with unremarkable imaging, are there any recent medical treatments?
Thanks Dr.Antony for your comment but avoidance of noisy situations may prevent deterioration of hearing loss and does not improve tinnitus. Do you have any experience about intratympanic injection of corticoids or lignocaine.
Unilateral tinnitus may be a presenting symptom in certain types of tumors like Acoustic Neuroma,Meningiomas,Please rule out the serious causes first.
Any since you didnot specify the age of the patient,In old age group Presbyacusis may present as tinnitus,in all age groups Chronic Rhinosinusitis can cause ET problem which may manifest as Subjective Tinnitus.
Mieneres disease may be one of the cause.
Please rule out Hypothyroidism and other systemic causes
Another important cause of unilateral tinnitus is TMJ problem,Please rule out the dental occlusion problem also.
In one patient I tried intatymapanic steroid with only partial improovement.
Regarding treatment tinnitogram can be done and patient may get benefit from tinnitus masker or a Hearing aid.
Repeated intra tympanic steroid injections can be tried
In my opinion it may not be induced by high noise exposure as this is a case of unilateral tinnitus and NIHL arre usually bilateral
Yes I agree that NIHL may occur unilaterally but in this part of the world it is commonly bilateral and associated with people working in mechanical industries
It is proposed that Tinnitus is caused by injury to OHC and probably it is a sign of aging nerve.
Thanks Dr. Akurati for your sharing but if you return to my question you found that I have already ruled out serious conditions like tumors by saying unremarkable imaging "Regarding unilateral tinnitus in a patient complaining of bilateral mild symmetrical high frequencies SNHL with unremarkable imaging, are there any recent medical treatments?" Do you have any research that prove that intratympanic steroid injection can improve tinnitus?
