In refractory sprue we need to rule out small bowel lymphoma and other conditions like coexistent IBS, lactose intolerance and small bowel bacterial overgrowth.Some researchers advocate the use of immunosuppressant in refectory sprue but the published data  in this field is limited. I would like to know the experience of colleagues in such cases 

probably only in refractory celiac type 1---unresponsive to prednisone, could also try in refractory type2 but not likely effective
